Palliative care is one of WundPal’s areas of service. The aim is to relieve distressing symptoms at an early stage, strengthen quality of life and provide clients and their relatives with continuous palliative nursing support in their own home. The presence of a wound is not a prerequisite.
When is palliative care from WundPal right for you?
This may be the case if you, your relatives, your patients or your clients recognise themselves in one or more of the following situations.
- You or someone close to you has been diagnosed with an incurable or life-limiting illness. Until the responsible mobile palliative care team becomes involved, you would like continuous interim palliative nursing support in your own home.
- You or your relatives are already receiving or have previously received care from WundPal due to a chronic or complex wound. A deterioration in your health has created an additional need for palliative nursing support that goes beyond wound care.
- Several healthcare professionals are involved in your care and your health condition is increasingly changing. You would like a consistent professional nursing contact person who provides reassurance, offers advice, maintains an overview, supports collaboration between everyone involved and accompanies you and your relatives over a longer period – if desired, until the end of life.
- You are discharged home with a tumour wound or another wound. Pain, wound odour, heavy exudate (discharge of wound fluid) or bleeding affect your quality of life and you would like an individual care plan aimed at relieving these symptoms – not simply regular dressing changes.
- Your medical and nursing care is ensured. Nevertheless, there is little room for the things that are personally important to you. You would like support that takes your individual needs seriously and works with you to find ways of maintaining or, where possible, improving your quality of life despite a serious illness.
- Questions about saying goodbye, meaning, hope or your personal wishes are increasingly becoming important to you, and you would like compassionate professional nursing support by your side in your own home.
What does WundPal specifically provide?
WundPal sees itself as a complement to existing palliative nursing care in the home environment. Cooperation with general practitioners, mobile care services, mobile palliative care teams and other healthcare professionals is an essential part of the support provided.
- Interim palliative nursing support until the responsible mobile palliative care team becomes involved
